We would like to invite you and your family to complete our Healthy Children, Healthy Families survey. The aim of the survey is to understand how you and your family living with you feel about health and wellbeing and how we can help you be as healthy as possible.
The survey is designed to be completed by you and your family, so please ask your parents or others who look after you to help you to fill it in.
Please note that you can’t exit and save the survey part way through… you must complete the survey in one go.
